To
enroll in a National Institute designation program, it is necessary
to first submit an application form, which can be downloaded from
this screen. To submit the form, please either mail or fax using
the contact information on the form. Please note that you may attend
one or more seminars or purchase self-study courses without first being
enrolled; however, no testing may be taken before application has
taken place.
The
points worksheet/related experience form and ethics statement may
be submitted at the time of application or at any time before the
designation program is completed. All enrollees must fill out the
points worksheet and ethics statement; however, the related experience
form need only be filled out if the points requirement cannot be
met another way. All of these forms may also be downloaded from
this screen.
